- HOSPICE PHYSICIAN works in conjunction with the patient’s primary physician.
- SKILLED NURSE manages the patient’s medical care and is highly skilled in assessing and managing pain and discomfort.
- CERTIFIED HOSPICE AIDE assists with personal care such as bathing, feeding, etc. Oversees dental and mouth care.
- SOCIAL WORKER assists in providing emotional care to the patient & family members, helps address financial issues, helps obtain any governmental support, if qualified, assists with end-of-life preparations such as funeral and mortuary arrangements.
- SPIRITUAL COUNSELOR/CHAPLAIN works in conjunction with the patient’s clergy, available to provide spiritual support for the patient and family available to conduct or attend memorial services.
- HOSPICE VOLUNTEER specially trained volunteers are available to spend time with the patient, offer companionship, and provide relief for the family.
- BEREAVEMENT SUPPORT offers compassionate care and guidance to individuals and families navigating grief. Providing a safe space for emotional processing, they offer tailored support to help cope with loss and build resilience.
WHO QUALIFIES FOR HOSPICE CARE
Patients whose physician determines that they may have a life-limiting illness and who choose comfort care qualify for Hospice. These patients include but are not limited to those having diagnoses such as cancer, end-stage disease, liver, kidney, heart, end-stage dementia, and ALS AIDS.
